I have never seen so fast progression of acetabular protrusion - even if his labs look good now, he had "bad infection" in the past (after External Fixation - by the way, I have near heard that somebody is treating femoral neck fractures with external fixator !!??).
I personally would consider him infected, planed to take prosthesis out, get deep cultures, put antibiotic spacer in, treat him with antibiotics for at least 6 weeks. If wound and labs would look good after, I would go back in approximately in 3 months, and would do impaction grafting of the acetabulum (he is very young, only 22y) and put new prosthesis in.
