After looking at the X-rays: Is it an idiopathic necrosis or a trauma result?
A grave picture I would say and I would doubt its only the AVN.
I would blame some additional causes.
I would suggest a Total Ankle(depending on the age and general condition) and consider a subtalar fusion in same session-after proper investigation I feel that the subtalar joint will be involved.
A pantalar fusion would be an alternative.
The thrombosis should be defined well and adressed.
