It really depends on how heroic you would like to be.  Assuming no infection... Total ankle is an option though I think she will wear it out
in 5 years or less given her age.  Fresh osteoarticular allograft could also be an option, you need to resurface both talar dome and tibial
plafond.  This too needs tempered enthusiasiam on the part of the patient, as they do also have a limited shelf life (hopefully 10 -12 years if she avoids impact activity such as running and sports).  The option of Total ankle arthroplasty is more attractive if she approached 40.  All that said, I think the best option is arthrodesis.
