I recommend curetting out the tumor, extending the margins with a high speed burr and then performing cryosurgery with liquid nitrogen. 2 cycles of cryosurgery are sufficient for giant cell tumor. To reconstruct the defect, I would place iliac crest bone graft under the subchondral bone surface (a small amount) and then fill the rest of the cavity with cement. I would also place small Rush rods prior to placing the cement to help with the reconstruction. If you do a literature search on giant cell tumor and cryosurgery, you will find a lot of literature on this technique.
