[Ortho] Застарелый вывих головки луча и парез лучевого нерва у подростка.

Здравствуйте! Это состояние известно как врожденный вывих головки 
лучевой кости. Вот что пишут об этом в книжках по детской ортопедии 
(Michael Benson Children’s Orthopaedics and Fractures 2010)  "Congenital 
dislocation of the radial head is a feature of numerous malformation 
syndromes, which should be sought during examination. The dislocation 
causes remarkably little loss of function during childhood, although 
radial head excision after skeletal maturity may be indicated for pain 
and prominence of the radial head if these symptoms are clearly 
attributable to the dislocation.
Dislocation of the radial head is liable to occur whenever growth of the 
ulna is diminished and radial growth continues. It may be possible to 
prevent dislocation if the length of the ulna can be maintained by 
distraction lengthening, with or without radial shortening or epiphyseal 
stapling. Parents may ask that the radial head be relocated. However, 
the indication for operative reduction of established congenital radial 
head dislocation has not been defined. The procedure may involve 
osteotomy of a bowed ulna and radial shortening in addition to 
reconstruction of the annular ligament. Considering the minimal symptoms 
experienced by most patients, such surgery is seldom justified".

Из собственного опыта видел всего один раз, тоже у подростка. Первые 13 
лет его ничего не беспокоило, на момент осмотра (14-15 лет) визуально 
определялась дислоцированная головка в виде подкожного образования по 
заднелатеральный поверхности. Имелось, не беспокоящее  пациента, 
ограничение разгибания в локтевом суставе и пронация предплечья. При 
этом парень живет нормальной полноценной жизнью. Думаю надо просто 
разъяснить Вашему пациенту и его родителям об этом состоянии, "о его 
уникальности" и разъяснить, что операция не гарантирует функциональный 
успех, подключить консервативное лечение.
