Alex,
What is her ankle range of movts.
The anterior spike seems to be more antero-lateral, rather than anterior, in such a position that it may not hamper dorsiflexion.
The ankle mortise as a unit is intact.
I would therefore only remove the anterior spike, if it impedes dorsiflexion.
If not, nothing at all.
