Dear Dr Maxim,
This is an interesting pathologgy and I myself WOULD NOT DO AN ASTRAGALECTOMY. I had such a case in an older male and treated it with a
pantalar arthrodesis with a graft. He does well, it was about 5 years ago.
The biopsy was very unspecific I used a reverese nail -you might use this of Smith &N or Aesculap, I find the best one of Stryker. I have seen one of Orthofix in publications, but did not do any. An Ex-Fix is a good option(circular frame). A POP will also do. I would put agraft after cleaning the surfaces of both joins. The incision I use-turn down the distal fibula.
A graft with consequtive ANKLE JOINT REPLACEMENT would fit, but is a longer option.
Good luck and thank you for sharing this case with us.
Plz pay attention that not whole the talus is hollow.
